file system structure excessively large and fragmented

we're getting issues creating files , setting security on 1 of our servers, getting error "the requested operation not completed due file system limitation"
in event viewer>applications , services logs>microsoft>windows>ntfs>operational, have error:

which assume why we're having issues.
however, can't find reference error on internet.
the drive in question fragmented , i'm running defrag suspect that's not going help. 'preventative maintenance' refers to? file needs defragging? can point contig defrag it?

sorry not replying sooner shortly after post disaster struck , lost security database, losing access drive completely. since we've been recovering it. think because tried various ways try defrag it, no avail. advice else error - don't try fix it. copy data off new volume.
no idea why error cropped noone else seems have ever been affected - 1.5tb drive 2 million files on it. user areas 2000 odd kids, each area own security settings. volume had been in use 5 years - it's on san , it's been moved old server @ point. deduplication enabled on it.
anyway, say, we're recovering data on drive should solve initial problem.
